USE THIS FORM IF:
You live in a Restricted State (CA, CT, IL, MA, NJ, NY or Washington D.C.) and wish to purchase ammunition from Bone Frog Ammunition but do not want the benefits of a membership.

Your submission here will require review by our team and you will be notified when you are approved to order.

CALIFORNIA: Direct shipment of ammunition to FFL holders or Active Law Enforcement only. LEOs submit your credentials along with your free membership submission here. If you are not an FFL holder you must provide a designated FFL to where we can ship your order.

INDIVIDUALS RESIDING IN CA: Submit your FSC and Government issued ID/Driver's License using the form below and follow instructions posted on our Shipping to Restricted States here. Your submission will not be approved without a designated FFL.

CONNECTICUT: Requirements - CT LTC and Government issued ID/Driver's License.

ILLINOIS: Requirements - FOID and Government issued ID/Driver's License. All ammunition shipments must be sent to the address listed on your identification. Orders placed with other shipping address will be canceled.

MASSACHUSETTS: Requirements - LTC/FID and Government issued ID/Driver's License. Orders placed with other shipping address will be canceled.

NEW JERSEY: FID and Government issued ID/Driver's License. Orders placed with other shipping address will be canceled.

NEW YORK: Orders sent to New York must be shipped to the care of a designated FFL, unless you are an FFL holder yourself. Please refer to our Restricted States page for more detailed information.

Once approved, your account will remain active as long as your credentials remain valid. Upon expiration of your credentials, you must email us your updated information.
